Good communities recognized by US News & World Report is hiring a Full-time Social worker. Under limited supervision, the Social Worker is responsible for providing clinical social work services to residents and families on an outpatient basis including counseling, crisis intervention, short-term case management, and information and referrals to community resources.

The Social Worker organizes, implements, and evaluates the Social Services program to meet the psycho-social needs of residents in the Health Care Center; work with residents in other levels of living when needed and also assist providers and Care Managers by acting as a consultant for residents with complex psychosocial needs.

Other key responsibilities include:

Support two LPC communities (Royal Oaks and Westminster Gardens).

Ensuring Health Care Center residents’ social service needs are identified in the care plan, that they are documented in the medical records, and that appropriate action is taken.

Developing and facilitating resident group activities, such as support, peer, grief and wellness groups with a therapeutic focus to meet the social and emotional needs residents.

Overseeing discharge planning for residents of Health Care Center to their home or other location, coordinating with therapy, nursing, physicians, family, home health agencies, etc.

Promoting and protecting the rights of each resident.

Performing other essential duties as required.

Status:

Full Time, M-F, 8am to 5 pm.

Compensation:

$55,210k - $80,054k Annually (DOE).

Qualifications:

Education– Masters of Social Work Degree or Licensed Clinical Social Worker.

Experience/Training– At least 3 years of experience working in healthcare setting, preferably with geriatric residents; or any equivalent combination of training and experience which provides the required skills, knowledge and abilities. Managed care experience.

Certificates, Licenses, Registrations– MSW or LCSW preferred.
