SUMMARYAssists with the coordination of team activities under the supervision of the Director of Clinical Care for the assigned team(s), provides secretarial services and other clerical functions by performing the following duties.

ESSENTIAL DUTIES AND RESPONSIBILITIESMaintain the hospice aide schedules.

Update schedules when patient status changes

Create schedules for two-weeks out from current date

Add new patients as directed by RN/CM verifying that visit frequencies and Hospice Aide plan of care are documented in Electronic Medical Record.

Ensure plans of Care (PPOC) and Certification of Terminal Illness (CTI) are signed by attending physician and or medical director. If attending physician is a non AH employee- these must be printed and sent for signature.

Verify that all necessary elements are completed on Election of Hospice Benefit (EHB).Scan required documents into the EMR as indicated. Uses agency standard scanning process.

Notification of patient death to all physicians listed in EMR and referral source. Document notification appropriately in EMR.

Verify EHB and POST form (if applicable) are in EMR when patients transferred to IPU.

Send care plan updates to the attending physician bi weekly throughout patients length of stay.

For Home Care, print orders as needed and send to appropriate physician for signature.

Forward discharge summary to attending physician for live discharges.

Prepare and distribute IDT meeting agenda.

Copy and distribute needed forms and other materials to IDT members.

Utilize copier/scanner, fax machine and phone system.

Order office supplies and forms for the team.

Maintain files of active patient records

Performs clerical duties for Director Clinical Care and IDT as requested.

Enter data and obtain reports from EMR system.

Scan and index patient paper documents to EMR daily. Print and fax orders to physicians as needed Register returned signed orders in EMR (home care). Follow up with physicians on orders not signed and returned.

Manage computer and projector of patient charts during team meetings or as requested by Clinical Director.

Prepare and mail bereavement cards.

Prepare recertification templates for physicians as needed

May perform receptionist duties as requested.

Other duties may also be assigned.

LANGUAGE SKILLSAbility to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

MATHEMATICAL SKILLSAbility to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITYAbility to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

CERTIFICATES, LICENSES, REGISTRATIONSIf required to drive to carry out the duties of this position:

current driver's license and automobile insurance as required by Tennessee State Law.

OTHER QUALIFICATIONSProficient in computer software systems using databases, spreadsheets, word processing, graphics, Desktop publishing, reports, charts, graphs. Project organization and documentation experience necessary.

Requirements

CONTINUING EDUCATIONThe agency requires this position is required to complete 6 hours of continuing education per year covering topics that will contribute to improvements in carrying out the above responsibilities. Regulatory agencies may require some disciplines to have additional hours in order to be licensed or certified.
