The Case Management Director will work in a fully accredited acute care hospital dedicated to providing the finest medical and health care services to their residents.

The Director of Case Management assumes responsibility for direct supervision of the Case Management department and coordinates the care of the patients across the continuum of care. The position is responsible for addressing utilization management, discharge planning activities in order to optimize patient outcomes within appropriate time frames.

The

Director Case Management will report to the Chief Nursing Officer, and will be responsible for directing daily operations of the Care Coordination departments with responsibility for both inpatient and outpatient services and oversight of Nurse Care Managers within this hospital system. The

Director Case Management will provide effective leadership and works closely with physicians, department staff as well as managers and staff of other departments to guide operational direction, planning and coordination of the department services.
