Position Summary:

The Ambulatory Care Navigator Coordinator II (ACN-C) is part of an interdisciplinary care team consisting of physicians, clinical professionals and social workers to assist in the cross continuum approach to hospital transitions. This position will be a key component to achieving collaborative practices that exemplify MultiCare Health System’s commitment to patient centered care, community engagement, and focus on transition of care for patients with physical and behavioral healthcare needs.

The Level II coordinator has significant behavioral health experience and/or is bilingual to be able to serve specific patient populations covered by MultiCare Health System.

Minimum Requirements:2 years behavioral health experience, OR bilingual in a population that is representative of the community we serve, OR has a Master?s degree in a health related field. Experience in utilization management, community resources, and/or financial and quality issues affecting patient care desirable

Community Health Worker experience desirable

Education/Licenses/Certifications:

Bachelor?s degree in a health related or behavioral health field, or an equivalent combination of education and experience in a health care or behavioral health setting
