Position Summary:

Under the direction of a Registered Nurse, the LPN Case Manager provides skilled nursing care for participants in the PACE Center, to include Adult Day Health and PACE participants. This position requires a service-oriented individual with strong interpersonal skills for the purpose of delivering and executing daily routine care and services.

Independent decision-making is expected in the execution of daily routine duties. Decisions regarding participant care or services require the review and approval of a Registered Nurse. Internal contacts include department team members from Adult Day Health and PACE departments.

External contacts include physicians, participants, families, caregivers, and visitors.

Minimum Requirements:

One (1) year experience working with frail seniors

Adult Day Health services experience preferred

Education/Licenses/Certifications:

Licensed in the State of Washington as a Practical Nurse

Possession of current CPR
