As a case manager, your primary responsibility is to provide comprehensive support and guidance to individuals in need in the City of Cambridge. You will conduct thorough assessments to determine clients' needs and develop personalized care plans to address their specific goals and challenges.

Your role will involve coordinating and facilitating access to essential services such as healthcare, housing, employment, and social support programs. You will monitor clients' progress, conduct regular check-ins, and make necessary adjustments to the care plans as needed.

Additionally, you will serve as an advocate for clients, collaborating with other professionals and community resources to ensure they receive the appropriate assistance and support to improve their overall well-being.

The ideal candidate for this position will possess the following skills, ability, and experience:

3-5 years of experience in providing case management, preferably with homeless adults or individuals unstably housed.

Knowledge of housing and housing-related services and resources, fair housing laws, and the ability to communicate effectively both verbally and in writing.

Fluency in Spanish is strongly preferred.
