Under limited supervision, the Care Case Manager helps post-acute patients and their families to navigate the skilled nursing rehabilitation experience. Coordinates with interdisciplinary team including nurses, therapists, and physicians regarding discharge planning.

Serves as liaison with managed care partners and communicates with patients about insurance resources and limits. Assesses and advocates for patient’s well-being during their rehab stay through care planning and coordination of services.

Full-Time Position $32.00 - $41.00/hr based on experience

Works closely with the long-term care social services designee to meet the psycho-social needs of patients and fulfill MDS requirements. Ensures regulatory compliance and proper documentation in electronic health record. The Care Case Manager will collaborate with the senior living team to support residents in any level of living who is struggling or transitioning levels.

To be successful in the role, you would have:

Current LVN/LPN

Current CPR certification

Prior LVN nursing experience within skilled nursing, senior care, post-acute or sub-acute care environments preferred

Experience working in healthcare/post-acute setting with focus on rehabilitation and/or any equivalent combination of training and experience which provides the required skills, knowledge and abilities. Managed care experience and geriatric focus preferred.
