Job Duties:

Ensures residents’ social service needs are identified in the care plan.

Develops and facilitates resident group activities such as support, peer, grief and wellness groups with a therapeutic focus.

Interviews residents and revises care plans as necessary.

Ensures social service assessments are completed for all new residents.

Assists Admissions including contact with hospital discharge planners, tours, coordination with nursing team, etc.

Documents social service needs as required by regulations.

Identifies and intervenes with high-risk residents requiring social services.

Promotes and protects the rights of each resident.
