Position Summary:
The Occupational Therapist will direct, supervise, evaluate and provide occupational therapy services to patients in the home as prescribed by the attending physician.

Position Duties:
Conduct Occupational Therapy assessment/evaluation to determine the patient’s level of function in areas including, but not limited to, strength, endurance, and ability to perform activities of daily living
Establish plan of care with functional and measurable goals within established time periods
Participate with staff and patient in prescribed therapeutic, self-care and creative activities directed toward improving independence as well as physical and mental functioning
Guide the patient with established household management routines
Instruct patients and families in the use of prosthetic, orthotic and assistive devises and the total occupational therapy program
Other Requirements:
Effective written and verbal communication skills
Must be able to speak and write in English
Unrestricted driver’s license
Current automobile insurance according to Company policy
Licensing/Certification/Education/Experience Requirements:
One (1) year experience as an Occupational Therapist
Home Health experience preferred
Graduate of an accredited occupational therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association or its equivalent.
Current occupational therapist license
Current CPR certification
Physical Requirements:
Physical Ability to lift to 40 pounds
Work requires walking, standing, bending, reaching, kneeling, pushing and pulling
